Healthy Futures HIU Officer (Derbyshire)


18 months fixed term contract

The successful candidate will contribute to the overall success of the Healthy Futures High Intensity User Project in Derbyshire. They will provide direct intensive, person-centred support to individuals with a history of poor health and disproportionate use of health and emergency services, to address negative and harmful behaviours. The post holder will employ a range of personalised support and coaching activities to achieve positive behaviour change and to build personal resilience.

The post holder will work as part of both the Healthy Futures team and the wider multi-disciplinary team associated with each client's support package. They will assist the Manager with the Healthy Futures Project function and collate appropriate monitoring data to measure the success of the service and report on this as requested.

  • Remuneration packages commensurate with level of experience
  • All candidates must be prepared to undergo a DBS check
  • A valid driving licence is essential for this position
