Healthy Futures shortlisted for Guardian Public Service Award

Healthy Futures is one of only two projects shortlisted in the Health and wellbeing category at the Guardian Public Service Awards.

The Awards aim to showcase the most innovative approaches that have been developed across public services, and create a forum for sharing new ideas and best practice.

Derventio’s Healthy Futures project acts as a bridge between hospital and home for patients with multiple complex needs such as homelessness, mental health or addictions. Since October 2013 we have provided services to over 330 patients and achieved some impressive outcomes for individual patients as well as the wider health economy.

We’re thrilled that the project has been recognised in this way and are looking forward to finding out the results on 29 November.
